  9. Compliance with the environmental and social safeguards policies of the Government and the Funders

  10. SEC registration, Articles of Incorporation and By-Laws

  11. Resolution of the Board of Directors and Sanguniang Bayan/ Panlalawigan authorizing the filing of loan application/transaction with DBP

  12. Local Government Unit's commitment to fund recurring cost (i.e. operations and maintenance cost)

  13. City/Municipal Business Permit

  14. List of machinery and equipment to be used by applicant with proof of capacity, ownership and/or mode of payment in case of lease arrangements

  15. Audited Financial Statements (last 3years)

  16. Income Tax Returns (last 3 years)

  17. Certificate of Borrowing Capacity from Bureau of Local Government Finance

  18. Customer Information Report (DBP Form)

  19. Customer Information Sheet (DBP Form)

  20. Other documents necessary in the evaluation of the loan
