PVAO Pensioner's Account Requirements:

For those residing in the Philippines

  • Two (2) copies of signature cards
  • Two (2) latest 1" x 1 " ID pictures
  • Two (2) valid and unexpired IDs with one photocopy each
  • Photocopy of PVAO Application Form with claim number
  • Bank Remittance for Active Pensioner
  • Letter of Introduction from PVAO
  • Customer Record Form
  • Minimum cash deposit of P100.00

For those residing abroad or pensioners who cannot come to the DBP Cash Management or Branch office

  • Two (2) copies of signature cards. For pensioners residing abroad, signature cards must be duly authenticated by the Philippine Consul General in the country of residence of the pensioner.
  • Two(2) copies of Special Power of Attorney (which must be not more than six months from execution date). For SPA executed by pensioners residing abroad, it should be duly notarized/authenticated by the Philippine Consul General in the country of residence of the pensioner and bearing the seal of the said Embassy

    The SPA should specify the following:

    • Purpose of SPA such as authority to secure blank signature cards and other bank forms to be delivered/mailed by the attorney-in-fact to principal pensioner and to open an account under the pensioner's name;
    • Relationship between the pensioner and the attorney-in-fact

    The designated attorney-in-fact should submit the following:

    • Two (2) specimen signature cards each with latest 1" x 1" ID picture
    • Two (2) valid and unexpired IDs with one photocopy each

  • Photocopy of PVAO Application Form with claim number
  • Photocopy of two(2) valid IDs duly certified/signed by the pensioner
  • Photocopy of passport duly certified/signed by the pensioner if residing abroad
  • Two (2) latest 2"x2" ID pictures
  • One picture (3R or 5R) of pensioner holding the latest issue of newspaper (broadsheet) showing clearly the date of issue
  • Bank Remittance for Active Pensioner
  • Letter of Introduction from PVAO
  • Customer Record Form
  • Cash deposit of P100.00 to open the account
